Job Description


  1. Provide home based family assessments, interventions, and collaborative treatment plans.

  2. Collaborate with other treatment providers working with the family, as well as key supports and resources in the family’s ecology.

  3. Engage primary caregivers and other key family members in active change-oriented treatment by identifying and overcoming barriers to engagement.

  4. Connect family members to community based, outpatient and inpatient substance abuse and mental health treatment providers, and other community resources as needed.

  5. Participate in weekly Clinical Diagnostic Team meetings, and bimonthly clinical staff meetings and trainings, as well as weekly individual supervision.

  6. Collaborate with relevant systems, including ACS, and key participants within each system to ensure effective collaboration and service provision.

  7. Assess safety and risk and implement crisis management plans and safety plans as needed, in collaboration with supervisor and Substance Abuse/ Mental Health Consultant.

  8. Maintain clear and concise documentation in case record, enter data in PROMIS.

  9. Other duties, as assigned.


Job Requirements

Master’s degree in Psychology, Social Work or Counseling.  Relevant license required, such as LMSW, LCSW, LMFC, and LMHC.  LCSW or equivalent degree preferred. 
Preferred Knowledge:  
Motivational Interviewing, Family Systems theory, CBT, Trauma informed care.
Preferred Experience:
Experience working with families, children and adolescents with substance abuse and mental health disorders preferred.  Experience working with families involved in the child welfare system preferred.
Reliable transportation required
New York Driver’s License required
