Job Description


The Coordinator has clinical and administrative responsibility, under the supervision and direction of the program director, within an AMI Apartment Program.  The Coordinator is responsible for ensuring the provision of services for residents, oversight of administrative processes, and supervision of apartment program staff.

KEY ESSENTIAL FUNCTIONS (List the functions that are essential to achieve the goals):


  • Provide weekly individual supervision of all assigned program staff.
  • Responsible for the provision of day-to-day services to program residents.
  • Provide direct service and/or coverage to residents consistent with restorative services and service plan goals.
  • Plan and participate in program activities.
  • Facilitate staff briefings, team meetings and case conferences.
  • Facilitate Consumer/Occupant Meetings.
  • Review and co-sign all assessments, individualized service plans and progress notes.
  • Complete required documentation and assist supervisees in completing documentation consistent with Medicaid, OMH and JBFCS requirements and guidelines.
  • Facilitate intakes and discharges.
  • Develop and maintain linkages with community service providers.
  • Conduct Utilization Reviews.


  • Participate in interviewing and hiring program staff.
  • Address staff performance issues in accordance with JBFCS Policy & Procedure.
  • Conduct staff training & development, as required, and conduct performance appraisals.
  • Schedule and monitor staff attendance and staff scheduling to ensure proper coverage.
  • Conduct regular apartment visits and walkthroughs.
  • Maintain relationships with landlords, handle leases, resolve apartment-related and facility issues.
  • Conduct Quality Assurance / documented chart review and audit.
  • Coordinate and oversee admission and discharge processes in accordance with all relevant regulations.
  • Prepare and submit monthly reporting and billing.
  • Submit monthly internal JBFCS reports.
  • Monitor program for continued improvement.
  • Participate in all meetings, as assigned.
  • Conduct outreach; maintain contact with referral sources; maintain census.
  • Work collaboratively as part of a service delivery team.


  • Provide and supervise crisis intervention services.
  • Ensure Crisis Intervention Event Reporting and follow up.
  • Provide on call administrative and crisis evening and weekend duties.


  • Other duties as assigned.

EDUCATIONAL / TRAINING REQUIRED (List all that is required to achieve this position):

  • High School Diploma, a minimum of 5 years prior supervisory experience, and a minimum of 5 years of experience working with adults who are mentally ill     OR Bachelor’s Degree, a minimum of 2 years prior supervisory experience, and a minimum of 2 years of experience working with adults with mental illness
  • Excellent case work, administrative, managerial and computer skills are required.
