Job Description

POSITION OVERVIEW:

Peer Specialist in the Adult Residential Department provides Adult Residential residents with training and assistance in basic living skills and provide other supportive services to help residents achieve and maintain their ability to live independently and realize their community integration goals.  Peer specialist serves as a role model and advocate on behalf of residents.

KEY ESSENTIAL FUNCTIONS (List the functions that are essential to achieve the goals):

  • Provide In Reach and utilize and reinforce the principle of recovery

  • Inspire individuals to become hopeful about their own recovery

  • Educate and inform the Adult Residential residence of choices

  • Help remove Stigma and Barriers associated with moving on to more independence.

  • Escort residents on tour of new neighborhood

  • Link individual residents to community resources in new neighborhood prior to moving

  • Provide in-home services to residents within the scope of program regulations and mission

  • Provide a range of direct services, including active, hands-on skills training and supportive counseling, targeted to help residents reach wellness and community integration goals

  • Provide instruction in general safety and security to all residents

  • Ensure that residents understand the programs occupancy requirements and assistresidents with meeting these requirements

  • Assist residents in discharge planning

  • Provide linkage and case management services as needed

  • Involve family members in service planning, as indicated

  • Escort and advocate for residents (court, school, hospital, benefits offices)

  • Attend staff meetings and trainings

  • Appropriately document services provided as required by program and regulatory agencies.

  • Participate in resident community meetings and facilitate assigned group services

  • Other duties as assigned

    EDUCATIONAL / TRAINING REQUIRED (List all that is required to achieve this position):

  • High School Diploma, GED or completion of Peer Specialist Training program required.Peer Specialist Training Certification is required.

    EXPERIENCE REQUIRED / LANGUAGE PREFERENCE (List what the desire to reach goals):

  • Relevant life experience preferred
  • Familiarity with NYS Entitlements, Housing and Mental Health Service Delivery System
  • Ability to organize, prioritize and execute responsibilities

COMPUTER SKILLS REQUIRED (List the computer skills needed):

  • Basic Computer Skills

WORK ENVIRONMENT / PHYSICAL EFFORT

  • Regularly required to travel to sites in and outside the agency using public transportation
  • Ability to recognize the need for confidentiality of information and maintain confidences.
