Job Description

OVERVIEW: The Jewish Board Care Management Programs help Health Home members and other eligible participants access and effectively use medical, behavioral health and social services that support their ability to live lives that are as healthy and independent as possible.  Implementing a person-centered recovery model to assist members’ pursuit of their health and wellness goals, Care Management staff work to minimize the occurrence of disruptive events such as hospitalizations, emergency room visits and institutional placements.

The Director has overall responsibility for the Jewish Board’s Bronx and Manhattan Care Management Services.  The Director is responsible for all aspects of collaboration with the Health Home and other regulatory entities.  The Director may supervise Administrative Supervisors, Supervisors, Office Managers, Consulting Psychiatrist, the Consulting Nurse and the Care Managers for each Care Management site as needed. As this is an evolving program, additional responsibilities will be added.

RESPONSIBILITIES (include but not limited to):

  • Lead the Jewish Board Care Management Programs in a manner that manifests the mission, vision and values of the Jewish Board.

  • Ensure that programmatic systems are in place and functioning to meet member needs.

  • Ensure that internal and external regulatory agency standards and requirements are met.

  • Provide individual supervision of the Supervisory personnel, Consulting Psychiatrist, Staff Nurse and Care Managers

  • Manage and adhere to program budgets, meeting productivity and revenue targets and monitoring expenses.

  • Oversee billing processes for program

  • Assist with the identification, development, implementation and completion of quality improvement projects and plans to solve problems/issues.

  • Facilitate and lead staff meetings.Participate in all meetings, as assigned.

  • Ensure intra-program collaboration and inter-agency collaboration including participation in linkage meetings with Lead Agency and other Health Home Care Management and service providers

  • Coordinate facility management, including maintenance, fire safety, security and renovation, as needed.

  • Perform chart reviews and ensure completion of appropriate and timely documentation.

  • Collect, analyze and report data, as required, and specifically regarding

  • Frequency of enrollee contact

  • Frequency of enrollee hospitalizations and ER visits

  • Staff productivity

  • Manage interviewing and hiring staff, as well as addressing staff performance-related issues in accordance with salient policies and procedures.

  • Assume on-call administrative and crisis evening and weekend duties.

  • Other duties as assigned.

Job Requirements


  • A graduate degree in Social Work, Nursing, Public Health, Public Policy or a related field
  • License in an appropriate field of practice (LMSW, LCSW, RN, NP, PA, MD)
  • Five years of supervisory experience in a medical or behavioral health setting
