Job Description

RESPONSIBILITIES include but not limited to:

  • Weekly visits with Youth to facilitate
  • Effective family communication 
  • Understanding of their family dynamics
  • Collaborative efforts of family, clinicians, educators and staff
  • Evaluation of need for referrals to community social services agencies
  • Sibling camaraderie
  • Discharge planning
  • Strength based family milieu
  • Planning and positive outlook for the future
  • Opportunity for youth to appropriately express their feelings
  • Progression with objectives around education, work readiness, and social adaptability
  • Understanding and cooperation with regard to the youth’s medication needs (where appropriate)
  • Provide connection to resources specific to clients’ diagnosis
  • Consult with Therapists, Psychiatrists, Case Planners, and Education Team
  • Attend case conferences
  • Maintain communication with Bio-parents and facilitate reintegration with the family of origin
  • Participate in Discharge Meetings
  • Attend trainings
  • Write Weekly Progress Notes
  • Facilitate Weekly Caring Families Group Meetings with Youth
  • Conduct outreach for youth who do not attend
  • Select topics and themes to address with youth
  • Maintain documentation of youth’s general progress and treatment while in the program, including: 
  • CAIRS is maintained and up to date
  • Service Plan Reviews are held monthly and documented appropriately
  • Collect and review the progress notes provided by the Family Based Treatment Parents, including all necessary medication monitoring forms
  • Regular fire inspections of FBT homes, and documentation of visits to FBT homes for follow up on youth progress with the FBT family
  • Complete Monthly reports and documentation for Medicaid billing
  • Work with team of family specialists on managing the emergency phone line after work hours (5pm through 9am, Monday through Sunday of week assigned.
  • Other duties as assigned


Job Requirements

:    Education/Credentials Required:  A Bachelor’s Degree in Social Science (Psychology, Social Work, etc.) with 4 years experience with youth with mental health issues, OR a Master’s Degree in Social Science with 2 years working experience with youth with mental health issues.
