Job Description

The Case Manager functions as a case planner and is responsible for supplying counseling and concrete service interventions to ensure appropriate client care. The case manager is also responsible for assessment and intake, providing therapeutic interventions and counseling.  The case manager is responsible for responding appropriately to crisis situations, and overseeing the range of a client’s services from assessment through to discharge and referrals for aftercare. Working within the client’s community, develops action plans to effectively identify and utilize existing resources and establish new resources for client care.   
KEY ESSENTIAL FUNCTIONS:
  • Evaluate clients for the purpose of beginning, monitoring and following up on individualized planning and protocols including child safety measures.
  • Individualizes and understands the needs of the individual client, the family and its support network in relation to each other.
  • Identifies appropriate treatment or service providers and facilitates client referral to internal and external and/or community linkage programs.
  • Performs short term individual/family counseling; Identifies and provides access to appropriate clinical and concrete services in the best interest of the client
  • Performs community outreach and establishes linkages with other groups, individuals or organizations which may be helpful to our clients.
EDUCATIONAL / TRAINING REQUIRED (List all that is required to achieve this position):
  • BA degree and a minimum of four years of experience in direct service.
  • Some specialized experience with specific client population or specialty (domestic violence, substance abuse, community outreach, etc.) preferred.
