Job Description

The Case Manager functions as Case Planner for all consumers in the Westchester DD Division. They will be responsible for coordinating Residential and Day Program treatment planning. They will facilitate all case planning conferences and prepare case conference summaries. The Case Manager will oversee the range of consumer services from assessment through to discharge from the program. The Case Manger will report to the WDD Director.                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                       

RESPONSIBILITIES include but not limited to:

  1. Evaluate consumers for the purpose of beginning, monitoring and follow up on person-centered planning.

  2. Individualizes and understands the needs of the consumer, their family and their support network.

  3. Serves as liaison between residence, families and day programs.

  4. Coordinates Residential and Day Program treatment planning.

  5. Chairs case conferences and prepares case conferences’ summaries.

  6. Monitors status of entitlements and follows up issues to ensure continuity.

  7. Pursue and monitor issues of guardianship, consent, registrations, applications and health care proxies.

  8. Provide focused case management services to consumers.

  9. Other duties as assigned.

Job Requirements

BA degree and a minimum of two years experience in direct service. Some specialized experience with specific consumer population.
