Job Description

POSITION OVERVIEW:

KEY ESSENTIAL FUNCTIONS:

  • Provide home based family assessments, interventions, and collaborative treatment plans.

  • Collaborate with other treatment providers working with the family, as well as key supports and resources in the family’s ecology.

  • Engage primary caregivers and other key family members in active change-oriented treatment by identifying and overcoming barriers to engagement.

  • Connect family members to community based, outpatient and inpatient substance abuse and mental health treatment providers, and other community resources as needed.

  • Participate in weekly Clinical Diagnostic Team meetings, and bimonthly clinical staff meetings and trainings, as well as weekly individual supervision.

  • Collaborate with relevant systems, including ACS, and key participants within each system to ensure effective collaboration and service provision.

  • Assess safety and risk and implement crisis management plans and safety plans as needed, in collaboration with supervisor and Substance Abuse/ Mental Health Consultant.

  • Maintain clear and concise documentation in case record, enter data in PROMIS.

  • Other duties, as assigned.

    EDUCATIONAL / TRAINING REQUIRED :

  • Master’s degree in Psychology, Social Work or Counseling.

  • Relevant license required, such as LMSW, LCSW, LMFC, and LMHC.

  • LCSW or equivalent degree preferred.

EXPERIENCE REQUIRED / LANGUAGE PREFERENCE:

Preferred Knowledge:  

  • Motivational Interviewing, Family Systems theory, CBT, Trauma informed care.

    Preferred Experience:

  • Experience working with families, children and adolescents with substance abuse and mental health disorders preferred.Experience working with families involved in the child welfare system preferred.
