The CCBHC is comprised of several community based programs providing direct care to over 3,200 clients annually, both on site and in the community. The Bridges to Care program is a brief treatment model designed to reengage clients in care who have stopped attending treatment and are deemed a risk to harm self or others or an elevated risk for hospitalization. The Program Supervisor Level II is a member of the program's  leadership team and is primarily responsible for implementation and oversight of new/innovative clinical services or models of care and clinical supervision and professional development  of licensed staff.   Additionally, this position is responsible  for ensuring clients receive timely access to quality clinical services; overseeing administrative functions to support program operations; and ensuring program  compliance with Federal, State, and City regulatory  bodies and funding entities.


RESPONSIBILITIES include but are not limited to:


Clinical Services:

  • Responsible for development and oversight of new clinical services or models of care to effectively meet the clinical needs of the individuals/families and communities we serve as well as ensuring clients have timely access to quality clinical care.  Duties are as follows:
  • Identify gaps in clinical services at the program level and develop potential solutions to fill the gap
  • Lead the implementation of evidence based treatment or have direct oversight and responsibility for innovative models of care, in partnership with JB training department and/or external agencies
  • Provide direct services to clients, as needed, including intakes and crisis interventions
  • Develop strategies to ensure clients have timely access to care and increased  engagement in care
  • Supervision of crisis intervention services
  • Implementation of quality improvement initiatives
  • Participate in quality assurance activities such as chart reviews and utilization management
  • Integrate recovery, rehabilitation, treatment and vocational components of the program
  • Monitor and follow up on program incidents


  • Responsible for ensuring appropriate staffing to support clinical care and evaluating staff performance.  Duties include the following:
  • Participating in hiring and orienting of new staff
  • Timely completion of performance appraisals and appropriately addressing performance concerns
  • Conducting or participating in staff training and development
  • Monitor staff attendance and vacation scheduling to ensure for program coverage
  • Supervision of staff, as appropriate to licensure



Administrative Functions and Program Operations: 

  • As part of the leadership team, responsible for ensuring program  operations are conducted efficiently; the program remains in compliance with relevant regulatory entities, and is fiscally viable.  Duties are as follows:
  • Facilitate team meetings, case conferences, and other meetings as assigned
  • Ensure program and staff are compliant with regulations and standards of care that govern the program
  • Utilize a variety of electronic databases to monitor billing and address any fiscal or billing issues that arise
  • As part of program management, identify and develop systems to improve program operations
  • May serve as back up to the Director or other supervisors; may assume responsibility for operations


REQUIREMENTS: Masters degree in human services and New York State clinical licensure in social work (LCSW). Solid clinical skills and knowledgeable of evidence based practices.  Excellent organizational skills, team work and communication skills.  Experience working with electronic medical records and proficient with Microsoft Office.
