Job Description


The Case Assistant  provides consumers who are diagnosed with psychiatric and substance abuse disorders with supportive counseling and case management. The program aims to help increase each consumer’s capacity to achieve psychiatric rehabilitation goals and maintain his/her sobriety/recovery from drugs and alcohol.  Case Assistants utilized their own recovery experience as consume or substance abuse and other behavioral health serves to support and empower others.

KEY ESSENTIAL FUNCTIONS (List the functions that are essential to achieve the goals):

  • Share and utilize persons experience as a recipient of health services to engage and support clients.
  • Provide a range of direct services, including supportive counseling, case management, and case coordination
  • Facilitate weekly  program groups
  • Help consumers identify, articulate, plan and achieve goals
  • As part of program team, reach out to service providers outside of the Bronx REAL Center for the purpose of identifying populations in need of MICA Self-Help support and developing linkages with provider agencies to provide said services
  • Complete all required documentation under the supervision of a social worker
  • Assist with intake and discharges
  • Provide follow-up services
  • Maintain contact with other service providers to coordinate service plans
  • Involve family in service planning and service provision, as indicated
  • Assist consumers in health-related goals, such as smoking cessation
  • Escort and advocate for consumers (court, school, hospital, benefits offices)
  • Provide crisis management
  • Attend staff meetings and trainings
  • Other duties as assigned.

EDUCATIONAL / TRAINING REQUIRED (List all that is required to achieve this position):

  • HS Diploma or GED  required- 
  • Experience working in interdisciplinary teams providing individual or group support to persons with chronic behavioral conditions

EXPERIENCE REQUIRED / LANGUAGE PREFERENCE (List what the desire to reach goals):

  • Ability and willingness to utilize one’s own experiences as a recipient of behavioral health, substance abuse treatment and / or chronic medical intervention services to perform job responsibilities and assist clients.
  • Must demonstrate interpersonal communication skills and the ability to emphasize effectively engage clients.
  • Ability to work independently and as part of services delivery team.
